Call for Nominations

Do you know an unsung hero? Someone who is doing good work in our community, who has not been recognized for their contributions and accomplishments? You can nominate deserving individuals to receive recognition at the 12^th Annual Celebration of Excellence Awards Gala, Saturday, April 25, 2009 at the Avalon Ballroom at the Fairplex in Pomona.

Inland Valley News, Inc., and its affiliated non-profit, public benefit corporation, Shining Glory Publications, Inc., welcomes your nominations of outstanding leaders in these categories:

Arts • Business • Civics • Community Activism • Community Service Education • Religion •Youth Leadership

Nominations containing all the information requested below must be received by Thursday, January 29, 2009 at: Inland Valley News, 308 N. Second Ave., Suite B Upland, CA 91786; phone: (909) 985-0072; fax: (909) 981-9998; email: ivnews@aol.com.  Those selected to receive Excellence Awards will be notified by March 1, 2009.  Recipients must be present at the Avalon Ballroom in Pomona the evening of Saturday, April 25, to receive their awards.

 Each nomination must include the following information, presented legibly:

1.) Nominee Identification:

• Nominee’s Name
• Mailing Address
• Phone numbers
• Email Address
• Company or Organization
• Category of Award for the Nomination (Arts, Business, Civics, Community Activism, Community Service, Education, Religion or Youth Leadership)

2.)  Nomination Rationale must include:

• The capacity in which you have worked or associated with the nominee
• 100 – 150 words describing the following:
  •  nominee’s outstanding accomplishments & why he/she is successful
  • obstacles the nominee has overcome to achieve his/her success
  •  impact of the nominee’s contributions to organization(s), business and/or the community (use quantitative terms, if applicable)
